None of us know what the future holds. By completing an advance directive, you and your loved ones can face the future with more clarity and confidence.
An advance directive form lets you name a healthcare agent—the person you want to make medical decisions for you in case you can’t make them for yourself. An advance directive also lets you describe the care you want (or don’t want) in certain cases.
