About the Program
The Extended Care and Rehabilitation Program at Fargo VA Healthcare System is composed of three areas —
- Physical Medicine and Rehabilitation
- Community Living Center
- Home and Community Care
Services Offered
Physical Medicine and Rehabilitation (PM&R)
PM&R services include —
- occupational therapy
- physical therapy
- speech therapy
- recreational therapy
- specialized services to veterans in need of rehabilitation following amputation, stroke, traumatic brain injury, orthopedic surgery and spinal cord injury
Community Living Center (CLC)
The CLC is a long term care unit, which maintains an average length of stay of 30 days. Most patients require extended rehabilitation and care following surgery and/or lengthy hospitalizations before returning to independent living. End of life/palliative and respite care for caregivers is also available.
Home and Community Care (HCC)
HCC consists of programs that manage care in veterans homes and community settings. Services include —
- home-based primary care
- adult day health care
- contract community nursing home care
- skilled and unskilled home care
- home hospice care
- home respite
Medical Foster Home Program (MFH)
The Fargo VA Health Care System is pleased to announce the development of the Medical Foster Home Program (MFH). MFH is a voluntary program that offers safe, long-term care in a home-like setting. MFH is an affordable, comprehensive care housing alternative for Veterans.
- Medical Foster Homes:
- Regular comprehensive case management visits are made by the VA Home-Based Primary Care Team
- Furnished private or semiprivate room
- Medication management
- Personal care (dressing, grooming, bathing, etc.)
- Meals, laundry, cleaning
- Maximum of 3 residents per home
- 50 mile radius from the Fargo VA Health Care System
- Medical Foster Home Caregivers:
- Must pass annual inspection by the VA Inspection Team
- Must have valid State foster care license to provide care
- Must be owned or rented by the caregiver
- Must be 21 years or older and financially stable
- Caregivers live in home and are experienced with providing daily hands on patient care
- Are physically able to provide the needed care
- Funding:
- Veteran is responsible for paying the caregiver
- Average rates are based on care needs: $1,500-$3,000/month
